POLICIES AND PROCEDURES
As a client you need to be informed of the policies and procedures of massage therapy and bodywork in order to provide informed consent prior to your bodywork session.
When you arrive for your appointment you will be asked to fill out a client intake form, which will be reviewed and discussed with you by your therapist. Please fill this out as accurately and to the best of your knowledge. This will ensure your safety as well as making sure that the therapist is practicing within their scope of practice. Be assured that your information on your intake form and anything you discuss with your therapist is kept confidential. Privacy and confidentiality are maintained at all times.
If the therapist feels your needs are outside of their scope of practice, they will refer you to your personal physician and have you obtain a doctors note before proceeding with your massage. The massage therapist is required to follow the recommendations of your physician.
It is important that you understand that your therapist can not diagnose or prescribe for any conditions or work outside of our scope of practice. Our time is very valuable to both the practitioner and the client so we ask that you be on time for your appointment. Sessions begin and end at the scheduled time. Sessions beginning late due to the client arriving late, will still end at the schedule appointment time and the full charge for the session will apply. We will do our best to address your needs with the remaining time. If the therapist is running late you will be extended time if possible or the price will be altered accordingly.
Your therapist will call 1-2 days prior to your scheduled appointment. If a cancellation is necessary it is requested that you give a 24 hour notice or you will be responsible to pay for your missed session. If you are sick with a cold, sore throat, stomach virus, anything contagious please reschedule your appointment. There is no charge for a missed sick visit that has been canceled.
Do not come under the influence of drugs or alcohol.
Sexual interaction or discussion of any kind between the client and the therapist is never appropriate. Sexual harassment is not tolerated . If the practitioner’s safety feels compromised the session is stopped immediately.It is important that the practitioner and the client respect each others boundaries at all times.
It is not recommended that you eat a heavy meal prior to you massage.
Be clean, shower the day of your massage.
Payment is expected at the time of your visit, your payment will be collected prior to your massage. If a check is written and is returned unpaid there will be a $25.00 return check fee.
Children under the age of 18 will not be seen without a parent or legal guardian present in the room.
After being shown to your therapy room. The therapist will step out momentarily allowing you to disrobe to your comfort level. A full sheet drape will be used at all times. Only the body part being worked will be exposed at that time.
I have read and agree to the policies and procedures and fully understand what I have read. Any questions or concerns I may have had were discussed and clarified by my therapist.
